We welcome referrals from Professionals within any sector, offering access to a wide range of services to promote and support health and/or social care plans.

Our holistic approach means that your client will receive wrap around support, providing them with access to information, help within their home, support following hospital discharge, working alongside them to find local community groups and activities that match their interests.

We provide a call-back service, after a client has finished receiving our support, to make sure they are happy, checking our presence provided the prevention or solutions that were being sought. This also provides us with the opportunity to give further assistance should it be required.
