Get support for yourself or someone else

How we can help

We provide support to individuals who may self refer, or to a friend, family member, or person you may be concerned about. We accept referrals from individuals and from professionals on your behalf.

Our person-centred approach means we start by listening to what’s happening and understanding what matters most to you. You don’t need to k

now which Care Network service you need before getting in touch – tell us a little about your situation and we’ll help you find the right support.

Depending on your circumstances, support may be available through our Help at Home, RISE or Community Support team. This could include practical and emotional support following a hospital stay or during a difficult period at home, help to maintain your independence, support with low mood or mental health challenges, rebuilding confidence, reducing isolation, preventing problems from reaching crisis point, or connecting with local services and community support.

We’ll work alongside you to understand your needs and priorities and identify the support that could help you feel more confident, independent and connected.

You can contact us directly

You don’t need to be referred by a professional to get support from Care Network Cambridgeshire.

You can contact us yourself, a family member or friend can get in touch on your behalf, or a professional can refer you. If you’re unsure whether we can help, please contact us anyway. Our friendly team will listen, talk through your circumstances and help identify the most appropriate next step.

Support may be available through:

Help at Home – practical and emotional support for adults returning home following a hospital stay, or going through a difficult period at home. We help people regain confidence and independence and manage safely at home.

RISE – community-based support for adults experiencing low mood, mental health challenges or other difficulties affecting their stability, confidence, independence or connection with others. Through practical and emotional support, RISE helps people take positive steps forward and aims to prevent problems from reaching crisis point.

Community Support – helping people access support, services and opportunities within their local communities. This includes our work with Community Car Schemes and other community-based projects and initiatives.

How to get support

Getting started is simple. Contact our team directly and tell us a little about yourself, where you live and what’s happening. You don’t need to complete a referral form or work out which service you need first – we’ll talk through your situation and help guide you towards the right support.

Are you a professional making a referral?

Cambridgeshire Care Network - Professional referrals

We also welcome referrals from health and social care professionals, social prescribers, housing organisations, voluntary and community organisations and other professionals supporting adults across Cambridgeshire and Peterborough.

Professionals can refer a client or patient to Care Network Cambridgeshire by contacting us directly, or through your own referral system such as JOY. 

If you’re unsure whether Care Network is the right service for your client or patient, please contact our team and we’ll be happy to discuss their circumstances and the most appropriate next step.

Follow-up support

Our support doesn’t necessarily end when a period of assistance finishes. Where appropriate, we’ll follow up to see how things are going, whether the support has made a difference and whether any further help could be beneficial.
