Help at Home Administrator
£24,011 for 37 hours a week Monday to Friday
Are you organised, outgoing and looking for a new challenge that makes a real difference to people when they need it most?
You will act as the primary support to our Help at Home Team, in receiving and triaging enquiries and referrals into our service, via our Inbox, telephone calls, internal referrals from other Care Network Teams, and through our involvement in the Voluntary Sector Alliance.
This exciting role will act as the first point of contact from both external and internal stakeholders.
You will be required to handle most of the referrals that come into our very busy Help at Home Service and to ensure that our team of Coordinators are kept up to speed on the referrals coming in, and that referrers know what has happened to their enquiries.
As such this is a very important role and you will be required to ensure that enquiries and referrals are dealt with effectively, efficiently and in a professional and positive way.
It requires a focused individual who can quickly complete tasks and can confidently manage their own workload and has the interpersonal skills to work confidently with a wide variety of audiences, from members of the public through to leading figures in the local NHS.
The post is predominately home based with at least one day at one of our offices at Chatteris or Hardwick per week.
We work with health and social care providers and other charities to ensure that whatever needs to happen actually happens so that people can stay in or return to their own homes and communities.

Community Navigator
18.5 hours per week. Salary: £13,381 (equivalent to £26,762 pro rata)
Our team of Community Navigators provides a gateway to information and services in each district of Cambridgeshire.
We are looking for someone to help people in East Cambridgeshire providing the information and guidance they need to live well. Through phone calls, face to face conversations and online support our aim is to encourage and enable people to do the things they need and want to do.
The person we are looking for will be organised and enthusiastic about working as part of a team bringing together voluntary services, community groups and statutory services. They will be able to keep online records and resources up to date and ensure that people get the information they need promptly.
This role is part of a job share, and ideally the successful candidate would be available to work Mondays and Fridays, as well as another day of the week (to be agreed).

Discharge Support Budget Coordinator
£26,762 for 37 hours a week Monday to Friday
Are you organised, have a keen eye for detail and looking for a new challenge that makes a real difference to people and to ensure that enquiries and referrals are dealt with effectively, efficiently and in a professional and positive way?
You will act as the primary support to the Voluntary Sector Alliance (VSA) in administering, recording and reporting on the allocation and spending from our Discharge Support Budget (DSB).
In addition, you will liaise with our current suppliers and source new ones to provide vital service support, to ensure patients are discharged effectively and that the likelihood of readmission is minimised.
This exciting role will act as the first point of contact for both external and internal stakeholders. You will be required to handle the referral requests to access the Discharge Support Budget, and to ensure that it is kept within the set budget, and stakeholders are kept up to date with spend allocation and monitoring of use.
You will ensure that clients are kept informed of the services provided via the DSB, and that they are aware of their responsibilities thereafter.
You will also be required to work closely with our Finance Manager to ensure that the DSB is kept on budget and that reports, and evaluation of spending is provided to key senior leads within the VSA on a regular basis.

About Care Network Cambridgeshire: For over 35 years we have been working alongside local people and communities across Cambridgeshire. We are passionate about making a difference to people and empowering them to live happier, healthier and independent lives.
About you: You will have leadership or management skills and want to contribute to a key local partner in the Health and Social Care sector. You will be curious to find out more about what we do and be able to help us do it better. You will be keen to contribute to focussed working groups tackling particular issues and be an ambassador for the charity in your local networks.
About our trustees: Our trustees serve voluntarily and pro bono; they have particular responsibility for strategy, quality, compliance, governance and oversight of the Leadership Team who manage the day-to-day operations of the charity.
