Wellbeing Community Support Coordinator

An interesting and varied role supporting adults in Peterborough

  • Could you help people to improve their wellbeing?
  • Could you combine this with recruiting and working with volunteers and liaising with other professionals, community groups, activities and organisations?

If so, we would like to hear from you!

You will be joining our caring, professional and flexible Wellbeing team, hybrid working, travelling to the office and Peterborough as necessary. Supporting clients over the age of 18 years, referred to Care Network via GP practices.

Care Network is an established charity with a strong ethos of staff wellbeing and support. We provide reliable technology to enable effective hybrid working, are a Living Wage employer, have signed the Menopause Workplace Pledge and are awaiting Carer’s Tick accreditation.

This post is funded for 12 months and is full time 37 hours a week (Monday to Friday, we do not normally work evenings or weekends). The salary is: £24,493 per year.

Discharge Support (Help at home) Coordinator

Could you liaise with hospital staff, charities, local authorities and others to ensure people are supported when they return home from hospital?

Are you organised, compassionate and decisive?

You will be required to work both onsite at Addenbrookes Hospital and in the community, supporting patient flow and discharges from hospital to provide the best client experience.  You will have a good knowledge of local and national services in order to help arrange wrap around care to our clients.  Care Network does not provide personal or nursing care though we can help to arrange these.

You will work closely with hospital teams and social workers to help people return home and avoid readmission. Essential to your role will be effective communication and decision-making skills as you coordinate the activities of other organisations to maximise the client’s wellbeing will be instrumental.

A pragmatic can-do approach is required and you can rely on the support of our team of staff and volunteers to make things happen.  Care Network is a supportive, caring employer.

37 hours a week (Monday to Friday,  we do not usually work evenings or weekends).  £24,983 per annum.
