Helping people in our community to live better lives
Our charity begins at home and in the community.
Our Mission is to support adults and communities to improve their health and wellbeing through a person-centred approach.
Services that support you to live well
Our services have been developed to empower people to access the care and support they need to live independent lives in their own home and make the most of interactions with their communities.
We support people across Cambridgeshire and Peterborough.
Need Help at Home ?
We support adults recovering from hospital, illness, or crisis at home
We provide a person-centred service with the aim of helping our clients to return home from hospital in a safe and timely manner, prevent an admission or readmission to hospital and make connections in their community.

We are so looking forward to trying some out and certainly we feel less ‘loaded’ and happier that there are safe ways for helping us to socialise.
It was great to get all that info sent to us. We are working our way through it. Mum has had contact with a social worker and we have sorted some groups for her to go to.
Everyone is so busy these days. I got just what I asked for and more. It was great and I think I’m doing as well as can be.
And I was ready to give up really as I missed my wife so much. Thank you for the lovely service you provide. I don’t like to go out, so it was nice to talk to someone.
Our entire family extends our thanks for all you have done.
